Originally Van is a separate dutch word meaning "from". But I believe it should be spelled with lower case in that original use. And of course if should be pronounced "fann". In the US it is often joined with the name into one word and pronounced "vann".

And to post even more interesting things: in contrast to the german "von" or the french "de" it has nothing to do with a noble title, but just means that a person is originating from a specific place, in Kevin Van Hooks case from a village or city called "Hook". Could be Hook of Holland, eh, Greg?
