Yeah, its Champions, The Superheroic Roleplaying Game.

The mechanics are the HERO System, Champions is just the Genre name. Champions/HERO System has been around a long time; it's currently in the 5th Edition. Here's the offical website: www.herogames.com

As far as the sheet, it's generated from Hero Designer, the official character creation utility for the HERO System. I wrote that particular export filter template however
