No, they have not. I think we were to assume it happened when he was originally killed amongst the crystal Forrest in A&A #0, but in A&A #1 there is no scar on Gilad's dead body.
It was also mentioned at one point that it was from the Immortal Enemy and thus why it wouldn't heal.

Yep, they haven't spilled the beans on that yet. I'm guessing they'll save it for a #0 issue! Can't wait to find out myself.

When we visited Valiant's office in 2012 after the NYCC, Dino said it was from the Immortal Enemy. That could have changed, editorially, by now, however.
