I think it symbolizes Pete breaking away from achieving a bearable state of mind with pharmaceuticals, ie Harada will teach him how to control himself without pills. Okay, the pill was Joe's but the foot is Pete's.

To me it looks like the dog's foot. 2 panels earlier the dog's foot is in the panel and I think it was put their to help figure out it's the dog's foot in this later panel. They way the toes curl also reminds me of a dog. Don't know the significance.

Zooming in on Comixology you can count 5 toes...a dog would only have 4 (and a duclaw). Also at that size it is pretty clear those are toe nails and not claws.
