I think it symbolizes Pete breaking away from achieving a bearable state of mind with pharmaceuticals, ie Harada will teach him how to control himself without pills. To me it looks like the dog's foot. 2 panels earlier the dog's foot is in the panel and I think it was put their to help figure out it's the dog's foot in this later panel. They way the toes curl also reminds me of a dog. Don't know the significance.

Zooming in on Comixology you can count 5 toes...a dog would only have 4 (and a duclaw).
