I remember when Shadowman Vol 3 #4 came out there was an issue with page order (kinda like the Magnus 57) what I don't remember is if it was only some or the whole run and Acclaim replaced the copies with corrected ones.

Not a printing defect in itself, but something that was created because of a printing defect (blank coupons).
