Outside of a convention or comic shop, if you want to mail a blank to a creator, then you’ll need.......

A signature series invoice, a artist agreement form, the sketch cover and the artist or the reps address. You’ll need to explain to them what you want on the cover, what to sign and where to ...

Yes. 100%. It has been stated before.
DMG will be listed as executive producer or something similar, but they had nothing to do with the movie, story, filming, etc.

I would mail them back to CGC though and get them in the new hybrid slabs. You don't want any damage developing over time. Meaning the high pressure on the sides warping the book.
